Job Description & Requirements Title: Physical Therapist – Home Health
Location: Melbourne, FL
Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00 – 17:00)
Provide physical therapy services to adult and geriatric patients in a home health setting
\tPerform comprehensive patient evaluations, assessments, and reassessments
\tDevelop, implement, and update individualized plans of care
\tEstablish and document short-term and long-term goals
\tComplete OASIS documentation accurately and timely
\tProvide discharge planning and patient/caregiver education
\tDevelop and instruct patients on home exercise programs
\tAddress safety, fall prevention, and injury prevention in the home environment
\tManage time effectively to meet daily visit expectations (approximately 7 visits per day)
\tDocument all care and progress using the Epic electronic medical record system
\tCollaborate as needed to ensure continuity and quality of care
\tRequired Skills & Qualifications:
Minimum of 1 year of physical therapy experience
\tHome health experience required
\tActive Florida Physical Therapist state license (must be in hand at submission)
\tCurrent BLS certification
\tEpic EMR experience required
\tPrior charge experience required
\tExperience with adult home health patient populations
\tStrong documentation skills, including OASIS
\tAbility to work weekends as required
\tTwo professional references from supervisory or leadership roles within the last year
\tPreferred Skills & Experience:
Strong time management and organizational skills
\tKnowledge of medications and patient safety practices
\tExperience educating patients and caregivers in the home setting
\tAbility to work independently while meeting productivity and quality standards
